I need a designer who can design something based on a concept ive created in forms. Im turning it into an app so need it designing properly. The forms link will give you an idea of what i want but im happy to answer any questions. Wellscore is a survey of 28ish questions broken down into 4 sections. I would like each section to work as a concertina whereas you answer a question then the next one comes down.
